Develop and implement individualized care plans with PTAs and nursing.
Continually assess and revise the PT care plan; collaborate with the multidisciplinary team.
Progress clients from discharge to goal attainment at home.
Provide education to patients and families.
Be part of a larger interdisciplinary care team with 24/7 clinical support.

🎯 Requirements

Active and current PTA license.
Graduate from a two-year PTA program approved by the APTA.
At least one year PTA experience under PT supervision.
Home health experience welcomed but not required.
Training provided and ongoing professional development.
