📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, strength, balance, and safety.
• Implement wound care and protective interventions as needed.
• Develop individualized care plans: exercises, gait, balance.
• Revise care plans with nursing in multidisciplinary team.
• Educate clients and families; document care in EMR tablet.

🎯 Requirements

• Current Ohio PT license.
• At least one year of PT experience.
• Graduate from APTA/CAAHEA/AMA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Proficient computer skills; EMR experience preferred.
