📋 Description

• Visit home health clients in designated geographic areas.
• Focus on orthopedic patient visits.
• Assess mobility, range of motion, strength, balance, and gait.
• Implement wound care and protective interventions.
• Develop and revise individualized PT care plans with the team.
• Educate clients and families on rehabilitation and safety.

🎯 Requirements

• Active and current Physical Therapy license.
• One year of clinical experience preferred.
• Home health experience welcomed but not required; we train you.
• Ability to travel locally to cases as assigned.
