📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostic tests and measurements: ROM, strength, balance.
• Implement wound-care interventions with evidence-based dressings.
• Develop and adjust individual care plans: manual therapy, gait, balance.
• Collaborate with nursing in multidisciplinary care plans.
• Educate clients and caregivers to promote health and independence.
• Document care observations and interventions using EMR tablet.

🎯 Requirements

• A PA Physical Therapist - PT license.
• Minimum one year of PT experience.
• Graduation from an APTA/CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
