- Make home visits to clients in designated territories.
- Perform mobility/ROM tests and other assessments.
- Implement wound care with evidence-based dressings and topical agents.
- Develop individualized PT care plans, including exercises and gait training.
- Assess and revise the PT care plan with nursing in the multidisciplinary team.
- Educate clients and families on rehabilitation, safety, and independent living.
Qualifications
- North Carolina PT license.
- Accredited PT program completion (APTA or CAAHEA).
- Ability to work independently and manage time.
- Strong interpersonal, organizational, problem solving skills.
- Solid computer skills; EMR experience preferred.
