📋 Description

• Visit clients in their homes to provide PT care.
• Deliver 1:1 home health PT and monitor progress.
• Conduct mobility, ROM, strength, balance, gait, and safety assessments.
• Develop care plans including manual therapy and therapeutic exercises.
• Educate patients and families on rehab care and independence.
• Document observations and interventions on a touch pad tablet.

🎯 Requirements

• Active Physical Therapist license.
• One year of clinical experience preferred.
• Home health experience welcomed; training provided.
• Ability to travel locally to cases as assigned.
