Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements: mobility, ROM, transfers, balance, strength, and safety.
Implement wound care and protective interventions with evidence-based dressings.
Develop and implement individualized care plans: manual therapy, gait training, balance, and other interventions.
Continuously assess and revise the PT care plan with nursing in the multidisciplinary team.
Educate clients and families in rehabilitative care for health and independence.

🎯 Requirements

Active and current PT license.
One year of clinical experience preferred.
Home health care experience welcomed, but training provided.
Ability to travel locally to cases.
