Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated territories.
• Perform diagnostics and measurements: mobility, ROM, stability, gait, strength, balance.
• Implement wound care with dressings and topical agents.
• Develop and implement individualized care plans: manual therapy, gait, balance.
• Continually assess and revise the PT care plan with nursing in the care team.
• Educate clients and families in rehabilitative care to promote health and independence.

🎯 Requirements

• Current Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA/CAPTE-approved program (transcript/diploma).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Strong computer skills; EMR experience preferred.
