Physical Therapist - Home Health Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, ROM, strength, balance, and gait.
• Implement wound care and protective interventions using evidence-based dressings.
• Develop and implement individualized care plans, including manual therapy, gait training, balance.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Accurately document observations, interventions, and evaluations using an EMR.

🎯 Requirements

• Active New Jersey Physical Therapist license.
• Minimum 1 year recent PT experience.
• Graduation from an APTA-accredited PT program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
