📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, gait, balance, strength, safety.
• Provide wound care and topical interventions as needed.
• Develop and revise individualized PT care plans with the team.
• Educate clients and families on rehab activities for health and safety.
• Document observations and interventions in EMR on a tablet.

🎯 Requirements

• Current NJ Physical Therapist (PT) license.
• Minimum of one year of recent PT experience.
• Graduation from an APTA- or CAAHEA-accredited program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
