Physical Therapist- Home Health

📋 Description

• Make home visits to clients in designated territories.
• Assess mobility, ROM, balance, strength; evaluate safety.
• Provide wound care and promote healing with dressings.
• Develop and implement PT plans with manual therapy, gait training, balance.
• Reassess and adjust care plans with nursing in the multidisciplinary team.
• Educate clients/family in rehabilitative care to promote health and independence.

🎯 Requirements

• Current DE Physical Therapist license.
• Minimum 1 year recent PT experience.
• Graduation from APTA- or AMA-accredited program (per transcript/diploma).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, problem-solving skills.
• Solid computer skills; EMR experience preferred.
