Make home visits to clients in designated territories.
Perform diagnostic tests and measures: mobility, strength, balance, gait, safety.
Implement integumentary protective interventions for wounds with dressings.
Develop and implement individualized PT care plans, including manual therapy and gait training.
Continually assess and revise the PT care plan with the nursing team.
Educate clients and families in rehab activities for safety and independence.

🎯 Requirements

Active and current PT license.
One year of clinical experience preferred.
Home health experience welcome, but not required; training provided.
Ability to travel locally to cases as assigned.
