Outpatient Physical Therapist, PT, Senior Living Visits (Sign On Bonus)

📋 Description

• Make outpatient visits to clients in designated territories.
• Perform mobility, ROM, balance and safety assessments.
• Develop and implement care plans including manual therapy and gait training.
• Continuously assess and revise care plans with nursing in a multidisciplinary team.
• Educate clients and families in rehabilitative care and independence.
• Document observations and interventions using an EMR system.

🎯 Requirements

• PA Physical Therapist license.
• Minimum one year PT experience.
• Graduate from an approved PT program (APTA/CAAHEA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.
