Outpatient Physical Therapist, PT, Senior Living Visits

📋 Description

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests: mobility, ROM, stability, gait, strength, balance, and safety.
• Develop and implement care plans: manual therapy, gait training, balance.
• Continuously assess and revise PT care plan; collaborate with nursing in multidisciplinary care plan.
• Educate clients and family members in rehabilitative care for independent living.
• Accurately document observations and interventions using a state-of-the-art EMR tablet.

🎯 Requirements

• PA PT license (license required)
• Minimum of one year of PT experience
• Graduation from APTA or CAHEA approved program
• Ability to work independently and manage time
• Strong interpersonal, organizational, and problem solving skills
• Solid computer skills; EMR experience preferred
