Occupational Therapist, OT, Senior Living Visits

📋 Description

• Make home visits to clients in designated geographic territories.
• Evaluate functional status including muscle function, endurance, coordination, and ADL/IADL.
• Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations to improve outcomes.
• Document observations, interventions, and evaluations using EMR on a touch-pad device.

🎯 Requirements

• A current DE Occupational Therapist license.
• Prior home health experience preferred, but not required.
• Minimum of one year of OT experience.
• Graduation from an OT program accredited by AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
