Make home visits to clients in designated geographic territories.
Evaluate patients' functional status and ADL/IADL performance.
Assess cognitive, psychological and motor performance; develop intervention plans.
Evaluate home environment for hazards or barriers to more independent living.
Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
Revise OT care plan and coordinate with nursing in the care plan.

🎯 Requirements

Active Delaware OT license.
Home care experience preferred, not required.
Min 1 year OT experience.
OT program graduation from AMA/AOTA-accredited program.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
