Make home visits to clients in designated geographic territories.
Evaluate patients' functional status including muscle function, endurance, ADL/IADL.
Assess cognitive, psychological, and motor aspects; create individualized plans.
Evaluate home environment for hazards and barriers to independence.
Identify equipment needs and environmental adaptations.
Update OT care plan; collaborate with nursing in the care team.

🎯 Requirements

A current DE Occupational Therapist license.
Prior home care experience preferred, but not required.
At least 1 year of OT experience.
Graduation from an OT program accredited by AMA/AOTA.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem solving skills.

🎁 Benefits

Base Pay, subject to negotiation: $73.00 - $76.00 per point.
$10,000 sign on bonus for full-time positions only.
