Occupational Therapist- Home Health

📋 Description

• Make home visits to clients in designated geographic territories.
• Perform evaluations of patients’ functional status, including ADL and IADL.
• Assess cognitive, psychological and motor aspects; develop intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Continuously update the OT care plan and coordinate with nursing.
• Educate patients and families in rehabilitative care to promote safety and independence.
• Document observations and evaluations in EMR using a touch pad tablet.

🎯 Requirements

• A VA Occupational Therapist license.
• Home care experience preferred but not required.
• At least one year OT experience.
• Graduation from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.
