Licensed Practical Nurse, LPN, Home Health Visits

📋 Description

• Making home visits to clients in designated territories.
• Administering medications, wound care, and therapies.
• Monitor client conditions; report changes to Clinical Manager.
• Follow up and document doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination; document care in EMR on a touchpad tablet.

🎯 Requirements

• Current OH Licensed Practical Nurse license.
• At least 1 year of recent LPN experience.
• Graduation from an accredited nursing program (transcript or diploma).
• Prior home care experience preferred, but not required.
• Ability to work independently and manage time effectively.
• Solid computer skills; EMR experience preferred.
