Act as internal case manager and advocate for Habilitation clients
Participate in Person-Centered Planning processes
Collaborate with client, family, Case Managers, and Hab Techs to implement plans
Make regular client visits to monitor progress and service delivery
Maintain ongoing communication with Case Managers and Care Coordinators
Supervise Habilitation Technicians per policy and state regulations

🎯 Requirements

Bachelor degree required
Must be a Qualified Professional with 1+ year post-degree I/DD experience
Prior supervisory experience a plus
Demonstrated record of increasing responsibility
Excellent communication and interpersonal skills
