Make home visits to clients in designated territories.
Perform diagnostic tests and measurements (mobility, gait, balance, safety).
Develop individualized care plans, including manual therapy and gait training.
Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
Educate clients and families in rehabilitative care to promote health and safety.
Document observations and interventions using a touch-pad tablet.

🎯 Requirements

PA Physical Therapist license.
Graduate from an APTA-approved program.
Ability to work independently and manage time.
Strong interpersonal, organizational, and problem-solving skills.
Solid computer skills; EMR experience preferred.
