📋 Description

• Make home visits to clients in designated territories.
• Conduct mobility and functional assessments (ROM, strength, balance, safety).
• Develop and implement individualized care plans with PT interventions.
• Continually update care plan with nursing in the multidisciplinary team.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in the EMR tablet.

🎯 Requirements

• PA Physical Therapist license.
• Graduate from an APTA-approved program or AMA CAA accreditation.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficient with EMR and computer skills.
