Provide clinical leadership, supervision, and ongoing support to field nurses and therapists
Oversee OASIS and HCHB workflows; ensure accurate, timely documentation and compliance
Review and manage care plans (485s) and clinical documentation to support quality outcomes
Lead onboarding, training, and professional development for field staff
Serve as a clinical resource by managing triage calls and guiding staff

🎯 Requirements

Active RN license
Home Health field experience required
Minimum 2 years of clinical nursing experience
Strong leadership, communication, and organizational skills
Strong, hands-on OASIS experience
