Associate Medical Director

📋 Description

• Provide assessment, planning, primary care, and continuous medical coverage.
• Collaborate with community-based physicians and IDT in discharge planning.
• Participate in IDT, communicate changes, and coordinate 24-hour care delivery.
• Maintain care of participants in nursing homes with regular visits.
• Take part in rotating night and weekend call.

🎯 Requirements

• Residency program accredited by ACGME
• Master’s degree or doctorate degree
• Board Certification – Internal Medicine or Family Practice
• Board Certification – Geriatrics, preferred
• Active California Physician License without restrictions
• Active CDS Certification and DEA
• Enrolled or eligible to provide Medicare/Medi-Cal services
• CPR and First Aid certification within 180 days of hire
