- Provide assessment, planning, and primary care for participants
- Collaborate with community physicians and IDT on discharge planning
- Participate in IDT; communicate participant changes and coordinate 24/7 care
- Maintain participant care in nursing homes with regular visits
- Rotate night and weekend on-call duties
๐ฏ Requirements
- Residency program accredited by ACGME
- Masterโs degree or doctorate degree
- Board cert: Internal Medicine or Family Practice
- Board cert: Geriatrics (preferred)
- Active California physician license without restrictions
- CDS Certification and DEA active
- Eligible/enrolled to provide services to Medicare and Medi-Cal
- CPR and First Aid within 180 days of hire (preferred)
