📋 Description

• Conduct MTM and DM via calls or in-person with beneficiaries.
• Discuss chronic conditions and meds to assess health impact.
• Educate on managing conditions and bioequivalent options.
• Collect medical history, medications, and lifestyle data.
• Promote medication safety and adherence; CMS compliance.
• Collaborate with cross-functional teams to deliver high-quality service.

🎯 Requirements

• Associate’s or Bachelor’s degree of Science in Nursing.
• Current and Active Nursing License.
• 3+ years of work-related experience.
• Knowledge of MTM/DM concepts (Asthma, Diabetes, Hypertension, CHF, Cholesterol).
• Excellent oral and written communication skills.
• Availability for on-site days in Puerto Rico and on-call hours as needed.
