- Provide leadership and performance management for RN, LVN, and MA staff.
- Oversee clinic function to address chronic and acute participant needs.
- Liaise with Complex Care for smooth post-discharge transitions.
- Develop workforce plans and staffing for diverse staff levels.
- Participate in direct participant care within the RN scope.
- Manage hiring, training, supervising, and mentoring of staff.
π― Requirements
- Unencumbered RN license required; BSN preferred.
- Minimum 2 years nursing experience with frail/elderly population.
- Minimum 1 year supervisory experience; mentoring staff preferred.
- Minimum 1 year of case management or care coordination preferred.
- Experience leading in a data-driven organization.
- Willing to work varied schedule, including nights/weekends/overtime.
