Provide leadership, oversight, and performance management to RN, LVN, and MA staff
Oversee clinic operations for chronic and acute participant needs
Liaise with Complex Care for smooth transitions of care
Develop workforce plans and staffing considering staff diversity
Participate in direct participant care within RN scope
Hire, train, supervise, and mentor direct reports

🎯 Requirements

Graduate of accredited nursing program with RN license; BSN preferred
Minimum 2 years nursing in clinical care for frail/elderly
Minimum 1 year supervisory experience; mentoring preferred
Minimum 1 year case management or care coordination preferred
Experience leading in a data-driven organization
Willing to work varied schedule including nights/weekends/overtime
