- Lead and supervise RN, LVN, and MA staff with clear roles and training.
- Manage day-to-day clinic operations and patient flow to ensure care quality.
- Oversee transitions of care with the Complex Care team post-discharge.

- Participate in direct participant care at the RN level when needed.
- Hire, train, supervise, and mentor direct reports to fit business needs.
π― Requirements
- RN license (unencumbered) required; BSN preferred.
- Minimum 2 years nursing experience with frail/elderly population.
- Minimum 1 year supervisory experience with mentoring.
- Minimum 1 year of case management or care coordination preferred.
- Experience leading in a data-driven organization using reports to prioritize.
- Willingness to work varied schedule including nights, weekends, overtime.
