📋 Description

• Lead, supervise RN/LVN/MA staff; ensure role clarity and training.
• Oversee clinic operations; address chronic and acute participant needs.
• Coordinate transitions of care with Complex Care team after discharge.
• Develop workforce plans and staffing for diverse experience levels.
• Participate in direct participant care within RN scope.
• Hire, train, supervise, and mentor direct reports; adapt to needs.

🎯 Requirements

• RN license required; BSN preferred.
• 2+ years nursing in clinical setting with frail/elderly.
• 1+ year supervisory experience; mentoring team preferred.
• 1+ year case management or care coordination preferred.
• Experience leading in a data-driven org using reports to prioritize.
• Willing to work varied schedule including nights/weekends/overtime.
