Identify, resolve, and prevent medication-related problems to ensure safe pharmacotherapy.
Conduct chronic disease management assessments under collaborative practice agreements.
Provide individualized counseling on medication use and lifestyle modifications.

🎯 Requirements

Doctor of Pharmacy (PharmD) and active Texas pharmacist license required.
Minimum 3 years as a clinical pharmacist; PGY1/PGY2 residency highly preferred.
Strong clinical judgment, communication, and process management.
Knowledge of FDA regulations and chronic disease management.
Ability to work on-site with interdisciplinary teams; comfortable with hybrid/onsite models.
