📋 Description

• Identify and resolve medication-related problems for safe therapy.
• Conduct chronic disease assessments under clinic protocols.

🎯 Requirements

• PharmD and active Texas license.
• Minimum 3 years as a clinical pharmacist; PGY1/PGY2 preferred.
• In-depth pharmacology knowledge and FDA regulations for chronic disease.
• Strong clinical judgment and communication.
• Ability to counsel patients and collaborate with interdisciplinary teams.
• Experience in medication safety and adherence improvement.
