- PharmD degree
- Licensed pharmacist in CA
- Plus any two of the following: Board certification in a relevant field; Postgraduate residency with direct patient care; At least 1 year of clinical services under a collaborative practice
- 3+ years clinical pharmacist experience or PGY2
- Strong communication, organization, and tech skills
- Empathy, integrity, and a team-first mindset
