Provide safe, efficacious, cost-effective pharmacotherapy; collaborate with ACP team on therapy management.
Conduct in-person or virtual assessments for disease-state management.
Identify, resolve, and prevent medication-related problems.
Support prior authorizations/appeals with clinical justification.
Coordinate care with physicians, specialists, and onsite pharmacists.
Promote patient safety and medication literacy through education.

🎯 Requirements

PharmD degree from an accredited school.
Licensed CA pharmacist.
3+ years as a clinical pharmacist (PGY1/PGY2 preferred).
Certification in a relevant practice area encouraged.
CA Advanced Practice license encouraged.
Strong communication and teamwork skills.
