Administer MAT injections; monitor adverse events; document lot/site/response in real time.
Plan daily routes and visits; maximize completed nursing visits and timely injections.
Perform field nursing visits: vitals, checks, wound care, COWS monitoring, vaccination.
Manage cold chain and supplies: transport, logs, inventory, sharps.
Coordinate with prescribers/CHWs/pharmacies to confirm eligibility and arrange deliveries.
Document thoroughly in EHR/case-management to maintain continuity.

🎯 Requirements

Active California LVN license in good standing.
BLS required.
Valid CA driver's license and insured vehicle; travel throughout LA County.
Organized self-starter who can independently plan day, routing, and timeboxing.
Comfortable providing direct care outdoors/in non-traditional settings; calm and solutions-oriented.
Spanish bilingual a plus.
