Overview
Akido is seeking a compassionate Community Health Worker to support residents of Bakersfield. This role focuses on connecting community members with essential health services, providing health education, and assisting with care coordination to improve health outcomes.
Responsibilities
- Conduct outreach to individuals and families in Bakersfield to identify health needs and barriers to care.
- Provide culturally competent health education on preventive care, chronic disease management, vaccination, and healthy lifestyles.
- Assist clients in navigating the healthcare system, scheduling appointments, arranging transportation, and connecting with community resources.
- Document interactions and outcomes in the patient records or CRM/EHR systems while safeguarding privacy and confidentiality.
- Collaborate with care teams, social workers, clinics, and community organizations to coordinate services.
- Monitor client progress, follow up on referrals, and track metrics to support program goals.
- Maintain accurate documentation and comply with relevant privacy regulations (e.g., HIPAA).
Qualifications
- High school diploma or equivalent; relevant health certification or coursework preferred.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work in community settings and travel within the Bakersfield area.
- Commitment to serving underserved populations and promoting health equity.
- Bilingual abilities are a plus; knowledge of local community resources is a plus.
- Familiarity with electronic health records (EHR) and data collection is desirable.
