- Perform Start of Care visits (1-3 per day) in a small territory.
- Deliver 1:1 skilled nursing and care per the individual plan (meds, wound care, treatments, comfort care).
- Assess patients, educate families, and monitor condition.

🎯 Requirements
- Active NJ nursing license in good standing.
- At least 1 year of related nursing experience; Home Health preferred.
- Experience with EMR; HCHB experience a plus.
- Experience with OASIS and Start of Care visits.
