- Perform 1-3 Start of Care visits per day in a small territory
- Deliver 1:1 skilled nursing, interventions, and care per plan
- Administer medications, wound care, and other treatments
- Conduct client assessments and educate patients and families
- Monitor patients’ condition and respond to changes
🎯 Requirements
- A current NJ nursing license in good standing.
- At least one year of related experience, preferably in Home Health.
- Comfortable utilizing an EMR; experience with HCHB a plus!
- Preferably experience with OASIS and Start of Care visits.
