- Perform SOC visits (Start of Care) and 1:1 patient care.
- Deliver 1:1 skilled nursing per care plan (meds, wound care, treatments, comfort care).
- Conduct client assessments; educate patients and families; monitor conditions.
- Work in home health with compassionate, reliable care.
- Document care in EMR and coordinate with care team.
🎯 Requirements
- Current NJ nursing license in good standing.
- At least 1 year related experience, preferably in Home Health.
- Comfortable utilizing an EMR; experience with HCHB a plus!
- Experience with OASIS and completing Start of Care visits.
- Strong critical thinking and communication skills.
- Ability to work independently in home health setting.
