Follow designated care plans per patient's needs.
Make home visits to clients in Cumberland, Salem, and Gloucester Counties.
Administer medications, wound care, treatments, and procedures.
Monitor client conditions; report changes to Clinical or Client Services Manager.
Follow up with and document doctors' orders.
Perform client assessments and case management.

🎯 Requirements

Current New Jersey RN license.
Minimum 1 year of recent medical-surgical clinical experience.
Graduation from accredited nursing program.
Ability to work independently and manage time effectively.
Strong interpersonal skills.
Solid computer skills; EMR experience preferred; ability to travel.
