Admissions Physical Therapist (PT)

📋 Description

• Make home visits for Start of Care (Admissions).
• Assess mobility, ROM, balance, and safety.
• Develop and implement individualized care plans.
• Revise PT care plan with nursing as needed.
• Educate clients and families to promote independence.
• Document observations using a touchpad tablet.

🎯 Requirements

• A current New Jersey Physical Therapist License.
• Minimum one year PT experience.
• OASIS experience required; HCHB preferred.
