Admissions Physical Therapist (PT)

📋 Description

• Visit clients at home for Start of Care (Admissions).
• Assess mobility, ROM, transfers, balance, strength, safety.
• Develop and implement PT care plans: exercises, gait training, balance.
• Update PT plan and collaborate with nursing on care plan.
• Educate clients and families on rehab for safety and independence.
• Document care using touchpad tablet.

🎯 Requirements

• NJ Physical Therapist License (current).
• At least 1 year PT experience.
• OASIS experience required; HCHB preferred.
