Perform mobility, strength, balance, and safety assessments.
Develop and implement individualized PT care plans (gait, balance, transfers, exercises).
Assess and revise the PT care plan; coordinate with nursing.
Educate clients and families on rehabilitative care.
Document observations and interventions using a touch pad tablet.

🎯 Requirements

A current New Jersey Physical Therapist License.
Minimum of one year of PT experience.
OASIS experience required; HCHB preferred.
