Make home visits for Start of Care (SOC) visits in Hudson County.
Conduct diagnostic tests: mobility, ROM, balance, gait, strength.
Develop and implement PT care plans: manual therapy, gait, balance.
Assess and revise PT plan with nursing in the multidisciplinary team.
Educate clients and families on rehabilitative care for safety and independence.
Document observations and interventions in client records on a tablet.

🎯 Requirements

New Jersey Physical Therapist License.
Minimum of one year PT experience.
OASIS experience required; HCHB strongly preferred.
