Ensure alignment between orders, patient needs, and documentation.
Audit nurse and therapy notes for proper documentation and consistency.
Identify discrepancies; follow up with clinical staff to resolve issues.
Support timely submission of docs to insurance providers.
Review referrals and ensure all required clinical docs and orders are in place.

🎯 Requirements

2-3+ years of relevant clinical experience
Registered Nurse (RN) or Licensed Practical Nurse (LPN) preferred
Experience in home health is highly preferred
Familiarity with OASIS assessments is a strong plus
Experience reviewing clinical documentation for compliance
Strong English communication skills (C1+ required)
