- Make home visits to clients in designated geographic territories.

- Monitor clients' conditions; report changes to Clinical/Client Services.
- Follow up with, execute, and document doctors' orders.

- Occasional on-call after hours and weekends.
🎯 Requirements
- Current VT and NH RN license.
- 1+ year of recent clinical (medical/surgical) experience.
- Prior home care or hospice experience preferred, but not required.
- Solid computer skills; EMR experience preferred.
- Ability to travel to cases as assigned.
